LMSW Social Worker Case Manager - Inpatient

Job Description:

• Provide all aspects of Social Work and Case Management for the assigned units (Inpatient psych unit may float to other units).
• Assist patients/clients in receiving services by telephone and in person.
• Responding for engaging patients/clients in beneficial activities.
• Assess patients and gather relevant information.
• Offer information and support patients/clients and their families.
• Responsible for contacting and making referrals to other agencies and services.
• Responsible for maintaining accurate records and preparing reports.
• Participate in training supervision and meetings.
• Watch for signs of child abuse.
• Provide crisis intervention.
• Perform other duties within scope of skillset/practice as assigned
