Job Title: Family Nurse Practitioner (FNP)

Salary: 110k to 130k

Job Description:

We are seeking Advanced Practice Nurses to join our progressive medical group practice in Oak Park IL. Enhance patient services in diverse clinical settings and be part of a growing patientcentered team. Join us!

Key Responsibilities:

• Commitment of 40 hours per week rounding at assigned facilities (SNFs Hospitals Clinics)
• Potential oncall duties some weekends
• Provide subacute care to patients at postacute care network SNFs
• Organize and deliver quality improvement and teaching services to nursing staff within client SNFs
• Assist with the development of High Acuity Unit(s) within the SNF setting
• Collaborate with a multidisciplinary team of primary care physicians and other allied clinicians to provide compassionate and highquality care within acute and postacute settings
• Develop patient care plans and work closely with nursing staff and patients families

Required Position Qualifications:

• Current unrestricted Advanced Practice Nursing License in Illinois
• NP national certification
• DEA License and IL Control Substance License
• Clinical background in adult (ANP) geriatric (AGNP) or family specialties (AFNP) (our practice age range covers adolescence to older adults)
• Proficiency in typing and computer skills
• Preferred past experience with SNFs and a solid understanding of postacute care services
• Interest in senior services
