Job Title: Home Health RN Weekend

Location: Carnegie PA 15106

Shift: Fulltime (40 hrs./week) Weekends Required

Salary: $42.00 per hour

Responsibilities:

• Manage and deliver patient care plans in the home health setting.
• Conduct patient assessments develop care plans and provide skilled nursing care to patients in their homes.
• Administer medications treatments and wound care as prescribed.
• Monitor patient health status and update care plans as needed.
• Educate patients and their families on disease management and treatment options.
• Collaborate with physicians therapists and other healthcare professionals to coordinate patient care.
• Document all patient care activities accurately and timely.
• Maintain confidentiality and comply with all regulatory requirements.

Requirements:

• Education: Associate degree in Nursing (Bachelors preferred).
• Licensure: Active Registered Nurse (RN) license in the state of Pennsylvania.
• Experience: At least one year of experience as an RN preferably in home health or a related field.
• Skills:
  • Strong clinical assessment and nursing skills.
  • Compassionate and patientcentered care approach.
  • Effective communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Excellent organizational and time management skills.
  • Proficiency in electronic medical records (EMR) systems.
• Availability: Oncall availability required during weekends.
• Travel: Local travel required to patient locations in Carnegie/Crafton PA and surrounding areas.
