Job Title: Hospice RN After Hours

Location: Irwin PA 15642

Shift: Fulltime (40 hrs./week) AfterHours Shifts

Salary: $65000.00 $75000.00 per year

Responsibilities:

• Provide compassionate and supportive care to hospice patients during afterhours shifts.
• Collaborate with the interdisciplinary team to develop and implement patient care plans.
• Assess patient needs manage symptoms and provide skilled nursing interventions.
• Support patients and families emotionally and educate them on endoflife care.
• Ensure timely and accurate charting and documentation of patient care activities.
• Coordinate with healthcare providers and community resources to enhance patient care.
• Adhere to hospice care regulations and maintain patient confidentiality.

Requirements:

• Education: Associates or Bachelors degree in Nursing (BSN preferred).
• Licensure: Active Registered Nurse (RN) license in the state of Pennsylvania.
• Experience: At least one year of experience in hospice or palliative care preferred.
• Skills:
  • Compassionate and empathetic approach to patient care.
  • Strong clinical assessment and critical thinking skills.
  • Ability to work independently and make sound judgments.
  • Effective communication and interpersonal skills.
  • Proficiency in electronic medical records (EMR) systems.
• Availability: Fulltime position requiring afterhours shifts and oncall availability.
• Collaboration: Ability to work collaboratively in a team environment.
