Job Title: Home Health Admission RN

Location: Mechanicsburg PA 17050

Shift: Fulltime (40 hrs./week)

Salary: $65000.00 $75000.00 per year

Responsibilities:

• Conduct patient admissions and assessments for home health services.
• Develop comprehensive care plans based on patient needs and assessments.
• Provide skilled nursing care including medication administration and wound care.
• Educate patients and families on home health care plans and selfcare techniques.
• Collaborate with the interdisciplinary team to ensure coordinated patient care.
• Monitor patient progress evaluate outcomes and update care plans as necessary.
• Document all patient care activities accurately and in a timely manner.
• Maintain confidentiality and comply with all regulatory requirements.

Requirements:

• Education: Associates or Bachelors degree in Nursing (BSN preferred).
• Licensure: Active Registered Nurse (RN) license in the state of Pennsylvania.
• Experience: Prior experience in home health community health or a related field preferred.
• Skills:
