About the Organisation:
The Nishkam Civic Association (Nishkam Centre) founded in 2002 is a valuesled organisation that strives for personal community and civic transformation through selfless service. The Macmillan Community Connectors project aims to reduce health inequalities through collaborative efforts ensuring culturally appropriate care for those affected by cancer. Working in partnerships with Macmillan Cancer Black Country ICB and Community partners

Job Responsibilities:

Offer basic telephone advice referrals and support services.

Listen to and understand the challenges faced by cancer patients seeking effective solutions.

Build relationships with partners in public private faith and third sectors for referrals.

Recruit and train community volunteers.

Build effective relationships across various sectors to support project goals.

Engage with partner organisations to raise cancer awareness.

Collect data for project evaluation and monitoring.

Promote the Macmillan Cancer project.

Deliver the project according to Macmillan Cancer Supports requirements.

Host information stalls and events in community settings.

Recognise and respond to acute situations appropriately.

Understand challenges around cancer to improve patient outcomes.

Provide support information on cancer care pathways.

Document and monitor all support and service activities.

Conduct occasional home visits to provide emotional and practical support.

Make planned telephone calls to assess patient needs.

Identify changes in need through telephone contact and respond accordingly.

Advocate for patients and address barriers to care.

Help patients access appropriate information and support.

Deliver selfmanagement information and support.

Encourage healthy lifestyle choices for cancer patients and carers.

Understand warning signs of concern in cancer care.

Maintain accurate records and case notes per policies.

Write progress reports for partners and leadership.

Work closely with the Programme Manager on stakeholder maps and key relationships.

Fulfil additional duties as required.

To take a proactive role in the identification of personal and group training and developmental needs which support NCA s objectives and to actively participate in the fulfilment of identified training and development needs.

To operate at all times within both the spirit and the practice of NCA Equal Opportunities policies.

19. Fluency in a South Asian language (e.g. Punjabi Urdu).
