Job Description

Job Title: Nurse Practitioner (Fayetteville VA ) Location: Fayetteville NC Organization: Fayetteville Veterans Affairs Medical Center (VA) Position Summary: AG Grace Inc is seeking a dedicated and experienced Nurse Practitioner to join our healthcare team. As a Nurse Practitioner at the Fayetteville VA you will play a crucial role in providing comprehensive healthcare services to our nations veterans. You will work collaboratively with physicians nurses and other healthcare professionals to deliver highquality patientcentered care to our veteran population. Key Responsibilities: Clinical Assessment and Diagnosis: Conduct thorough patient assessments including physical examinations medical histories and diagnostic tests. Formulate differential diagnoses and develop appropriate treatment plans in collaboration with the interdisciplinary healthcare team. Treatment Planning and Management: Develop and implement evidencebased treatment plans for acute and chronic medical conditions. Prescribe medications order diagnostic tests and perform minor procedures within the scope of practice and in accordance with VA regulations. Health Promotion and Disease Prevention: Educate patients and their families about preventive healthcare measures lifestyle modifications and selfmanagement strategies to optimize health outcomes. Promote wellness initiatives and participate in community outreach programs. Collaborative Care Coordination: Work closely with physicians nurses social workers and other healthcare professionals to ensure seamless coordination of care across various departments and specialties. Participate in interdisciplinary rounds case conferences and care coordination meetings. Documentation and Electronic Health Records (EHR): Maintain accurate and uptodate patient records using the VAs electronic health record system. Document all clinical encounters assessments treatments and patient education activities in compliance with VA standards and regulations. Quality Improvement and Compliance: Participate in quality improvement initiatives performance improvement projects and clinical audits to enhance the delivery of healthcare services and ensure compliance with VA policies protocols and regulatory requirements. Professional Development: Pursue ongoing professional development opportunities including continuing education training seminars and certification programs to stay abreast of advances in nursing practice healthcare technology and VAspecific policies and procedures.
