Job Description:
Job Title: Program Specialist (Social Worker) 1
Location: Remote in TX
Pay Rate: XX25.00 Per hr.
Duration: 6 Months (Possible Extension)
Shift: 8AM 5PM (MonFri) (CST)
Job Summary:
- Conduct screening and risk assessment interventions per program guidelines.
- Assist with nonmedical aspects of the members care including referrals to community resources.
Must Have:
- Social Worker Liscence in TX.
- Travel Required 5%10%. ((Just in case of teambuilding events or meetings))
Job Responsibilities:
- Identify special needs members through the completion of health screens and other resources Work with community outreach/member advocates to coordinate member care.
- Educate providers and community resources on program components and available support services.
- Must be able to meet strict turnaround times with documentation.
- Conduct follow up calls in a timely manner.
- Flexible and Professional
- Complete assigned tasks and document findings
- Coordination with multidisciplinary teams
- Follow up calls to members.
- Any other projects that may arise
- Educate members with special needs to foster compliance with program and positively impact outcomes.
- Conduct site visits as appropriate for programs and provide support to other special programs.
- Develop and modify care plans in conjunction with members members family and managing physician.
- Development of plan specific literature and education materials in conjunction with medical director and corporate oversight.
Education/Experience:
- Bachelors degree in Social Work Nursing Health Behavioral Science or Equivalent Experience.
- Experience in a managed care environment preferred. 02 years of experience
