Weekend Hours: Weekend rotations are every 4th weekend with a set day off/before after.
Call Requirements: Our oncall shifts are rotated during the week and on the weekends. Weekday oncall would start at 5PM and end at 8am. Occasionally there are prn visits that are needed overnight but these are rare as the nursing supervisor is able to triage and direct patients over the phone. Weekends begin Friday at 5PM through Monday at 8am. All of the oncall requirements are rotated through all staff nurses. This is estimated at 1x/week and 1 weekend/per month
Years of Experience: must have at least 1 year home health/hospice experience

Unit Information:
Patient Ratios: Case Manage 1418 patients (based on acuity) with an expectation of 56 visits per day. RNs should be able to see 56 patients per day or 56 units per day. Minimum Productivity is 25 units per week. SOC 2 units; Recert 1 unit; DC 1 unit.
Equipment: clinician will be assigned an agency laptop and an I phone with case and chargers mouse a nursing supply bin nursing bag with vital signs equipment and an agency bag. All must be returned upon departure.

Preferred Skillset/Experience: PICC lines/dressing changes trachs port access ostomies chest tubes Pleurex drains S/p catheters lab draws/UA/C&S etc.
Type of Patients: Adult med surg/oncology/Cardiac/Neurological/Respiratory/ Wound Care/GI/GU patient
Personality Fit: Collaborative team approach good communication open to feedback to improve practice or workflow
Dress Code:
Scrub Color: business casual or scrubs (any color).
Any unacceptable attire: No jeans open toed shoes or clothing with holes
Misc./Other Information:
Must be able to successfully case manage/selfschedule your patients.
Use of inhome infusion pumps. No ventilators. No invasive monitoring.
