This is an outstanding opportunity for a mission driven team player who is passionate about enabling seniors to meet their goals and live the independent lives they deserve. In this role you will serve on an interdisciplinary team (IDT) and provide direct medical care to PACE participants. You will work collaboratively with the IDT to create and drive forward a care plan that improves health outcomes for your panel of participants. In this role you will meet participants where they are whether that be in the center clinic home virtually or an external site of care.
Key Responsibilities
Provide direct medical care to PACE participants
Complete sections of assessments and care planning
Manage a panel of participants
Initiate evaluate review and change orders for medications and diagnostic studies or treatments with approval
Assess initiate and coordinate care on a daily basis under supervision
Document all participant interactions assessments next steps in the EMR within the specified timeframe set by the center
Make referrals to specialists with approval and request consults as needed
Perform history and physicals and participant teaching
Order interpret and follow up on lab tests and results
Perform selected procedures
Recognize and escalate abnormalities as needed
Liaise with other team members for participant s care including behavioral health and pharmacy
Evaluate and treat participants where they are located whether that be in the center home virtually SNF etc
Facilitate advanced planning discussions with participants
Document chronic conditions via HCC capture
Schedule and Shift Details
40 hours per week no oncall. Schedule will vary.

qualifications
required- Masters degree or higher
- Nurse Practitioner (4 years)
- 4 years of experience in ANY of the following:
- Geriatric Care
- Geriatric/LTC
- Valid American Heart Association Basic Life Support (AHA BLS)
- Valid Nurse Practitioner (NP) in the state of California (CA)
