Trinity Health PACE of Montgomery County with Holy Cross Health are seeking a Board Certified or Board Eligible Geriatric Medicine physician to join our Asbury Health Partners team located in Gaithersburg Maryland.
Job Description:This is a new PACE location to open in summer/fall 2024.Asbury Health Partners provides easily accessible highquality primary care services to independentliving residents at Asbury Methodist Village and community members aged 50 and older. The 2739squarefoot primary care site features the latest technology stateoftheart examination rooms and a staff of experts in senior care.The practice is conveniently located on the Asbury Methodist Village campus allowing providers to coordinate care and recommendations with oncampus services such as wellness rehabilitation assisted living and skilled care. Lab and ECG services are provided onsite. Specimens or blood samples that need to be tested are sent to Holy Cross Hospital.Office hours are Monday through Friday from 8:00 am 4:30 pm. The practice provides 24hour oncall coverage to ensure that Asbury residents are able to reach a skilled provider regardless of when a health issue arises.This is an excellent opportunity for the right Geriatrician to work with a dynamic team of clinical professionals at a wellrespected healthcare organization. Provides general medical direction and supervision of the medical aspects of participant careResponsible for the delivery of participant care clinical outcomes and oversight of the Quality Assurance Performance Improvement (QAPI) program. Responsible for the development and maintenance of medical policies and procedures/standards and protocolsProvides direction and supervision to related Primary Care Providers (PCPs) and APPs.Provides leadership and guidance to the program colleagues and providers in the local community
Need to have a drivers license proof of insurance and means of transportation
Proficient in Microsoft Suite (Word Excel PowerPoint etc.) in addition to basic knowledge of email calendaring systems and other software as required to perform the essential functions of the jobExcellent organizational customer service flexibility and problemsolving skillsWillingness to travel within the region periodically as neededConduct history and physical on new participants and eventbased and periodic reassessments. Develop medical treatment plan and provide medically necessary services in the range of locations in which participants may reside.Interview and evaluate applicants to PACE Organization and their caregivers and participate in the Intake and Assessment decision.Discuss each participants medical conditions and prognosis with him/her and their caregiver to assist them in expressing their wishes and planning their future care.Execute assignments in a culture that is shared and collaborative across all divisions within TH PACE.Collaborate in the nursing treatment program of participants.Establish and/or maintain as a member of the leadership team a plan to achieve operating goals.Participate as an active member of PACE Organization interdisciplinary team. Educates and advises the team regarding the implications of diagnoses cognitive and physical function and treatment options.Participate in team and family conferences and decisionmaking processes.Educate participants and families regarding health maintenance and their chronic medical conditions in cooperation with nursing staff.Supervise the health care and treatment program as developed by the interdisciplinary team.Perform physical examinations order and evaluate appropriate laboratory and diagnostic tests and record results in a systematic manner.Effective skills in physical assessment and chronic disease management for frail older adults.Desire to work in a creative alternate health care setting.Able to manage changing priorities according to participant needs.Participates in provider oncall rotation as required.Minimum of 1 year experience working with the frail or elderly population is required.

- Board Certified in Internal Medicine or Family Medicine REQUIRED

- MD license (or willing to obtain) REQUIRED
- Clean Malpractice HIGHLY PREFERRED
- CDS Certification (by time of start) REQUIRED
