Position Title: Physical Therapist
Position Summary:
The Physical Therapist plays a crucial role in restoring function and minimizing disability following disease or injury for our clients. Responsibilities include evaluating and treating patients using medically prescribed physical therapy programs as well as supervising Physical Therapy Assistants according to agency and regulatory standards.
**Areas of Responsibility:**
Conduct initial admission/evaluation visits including comprehensive assessments and documentation.
Initiate regularly reevaluate and revise plans of care in consultation with the Manager of Clinical Services.
Provide and document skilled care/interventions on all visits including skilled observation medical procedures and patient/family education.
Assess the need for services from other team members and coordinate care accordingly.
Document observations clinical findings and treatment plans accurately and timely.
Supervise Physical Therapy Assistant(s) and ensure adherence to treatment plans and documentation requirements.
**Requirements:**
Graduate from an accredited physical therapy program approved by the Council on Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association.
Active California license as a Physical Therapist.
Minimum 2 years recent experience as a Physical Therapist preferably with experience in gerontology and home care.
Current CPR Certificate.
Current California drivers license and automobile insurance.
Demonstrates willingness and ability to work collaboratively in a multidisciplinary team environment.
**Essential Physical and Mental Requirements:**
Must be able to drive to patients homes and maneuver around various environments.
Must have clear sensory abilities for effective communication.
Must possess clinical knowledge and critical thinking ability to provide patient care consistent with standards and regulations.
**Additional Information:**
This is a fulltime position with hours from 8:30 AM to 5:00 PM.
Must have an active automobile ID and insurance with bodily injury coverage.
Home care/homebase experience outpatient or clinic experience and nonprofit experience are preferred.
