The Staff Pad has partnered with a community health center in Nogales Arizona. We are looking for a Psychiatric Clinical Pharmacist to join this amazing healthcare organization. The Psychiatric Clinical Pharmacist will function as an engaged member of the healthcare team to facilitate the development implementation and continuous improvement of programs and systems to maximize patient satisfaction within a culturally sensitive patient centered care plan. The Psychiatric Clinical Pharmacist will also coordinate educational activities and actively instruct all students enrolled in experiential pharmacy practice rotations at our clients pharmacy.
Responsibilities:
- Ensure safe appropriate costeffective medication therapies which adhere to established accepted clinical guidelines approved treatment protocols and internal patient care policies.
- Serve as a source for unbiased drug information and education specific to pharmaceutical care drug therapy and medication therapy management for providers caregivers staff members and patients.
- Assist providers and chronic disease managers with creating and managing complex drug regimens and measuring outcomes.
- Assist Manager of Clinical Pharmacy Services on the review formulary products to provide pharmaceutical stewardship to better serve our patient population.
- Perform Medication Reconciliation Assessments as needed on hospital/emergency room/urgent care discharges and as requested by clinic providers.
- Review and monitor medication therapy regimens for allergies adverse drug reactions drugdrug interactions therapeutic duplications contraindications and inappropriate medication dosing.
- Develop workflows protocols and standards of practice for medication reviews by staff pharmacist.
- Develop medication therapy assessment tools for staff pharmacists when clinical pharmacist is unavailable for consultation.
- Maintain competence and suitable continuing education as required for position.
- If requested by provider or Care Coordination assist by provision of direct patient care services via home or facility visit. Provide immunization services.
Requirements
Educational Requirements:
- Bachelor of Science Degree in Pharmacy mandatory if not Pharm.D.
- Advanced degree preferred (PharmD).
- Accredited Post Graduate Residency is required.
- Board certification by Board of Pharmacy Specialties for Psychiatry is required.
Experience:
- Minimum 3year suitable experience as a licensed pharmacist.
- Must secure an adjunct faculty appointment as a Clinical Instructor of Pharmaceutical Practice with the University of Arizona College of Pharmacy within 4 months of start date.
Certification/Licensure:
- Must hold unrestricted license by the State of Arizona and Department of Public Safety Fingerprint Clearance Card.
