Job Description
We are seeking a full-time Therapist (MSW or Licensed), with experience providing clinical care for individuals challenged with addiction.
Responsibilities:
Provide individual therapy on a consistent basis with assigned clients, utilizing biopsychosocial assessments, treatment plans, and progress notes, according to company policies and procedures to ensure the highest quality of care
Plan and facilitate group therapy, according to the clinical schedule, that is understandable by clients and aligns with treatment plans to ensure effective services are provided
Perform psychosocial needs assessment and develop treatment plans for new patients
Monitor and evaluate effectiveness of care plan and modify plan as needed with appropriate documentation in progress notes
Interact continuously with client, family, physician(s), and other resources to determine appropriate behavioral action needed to address medical needs.
Develop and implement appropriate discharge planning with clients, family, and facilities/agencies
Research community resources and train/create behavioral routines that helps enable clients to be active participants in their own healthcare.
Requirements
Minimum 2 years of experience providing direct care for individuals diagnosed with addiction
Must be able to pass a criminal history and drug test upon position offer
