The Staff Pad is partnering with a new Behavioral Health Clinic in the Lewis-Clark Valley. We are hiring a Behavior Health Navigator to
join the growing team. Our client is a non-profit whose mission is to improve the overall health of their community by expanding access to quality health and wellness services. They hope to fill the void within the community by offering mental health services to everyone, regardless of insurance status.
Responsibilities:
- Participate as directed in the screening of new patient requests to ensure that all individuals are adequately and appropriately serviced according to their individual needs
- Develops and regularly reviews appropriate treatment plans with clients to implement appropriate services and developing effective alliances with clients to achieve more effective personal, social, educational, and vocational development
- Provides ongoing supportive and/or case management functions in accordance with problems, needs, and strategies identified within the treatment plan in order to help the clients to achieve the stated goals and objectives.
- Works collaboratively with patients to access health services, and outside entities, facilitate effective transitions of care for hospitalized patients, arrange housing, transportation, and address other social determinants of health.
- Maintains close communication with Psychiatric Prescriber for input regarding medication compliance, side-effects, and alerts staff of client change of care needs.
- Provides or facilitates at-home or community-based visits with patients as needed.
Requirements:
- Bachelors Degree in Human Services field with 3 years of experience in a Behavioral Health Setting preferred.
- Effective Oral and Written communication skills required
- Detail-oriented, organization, leadership, and self-motivation required.
- BLS, CPR, and AED required.
- Commitment to supporting a safe, respectful, equitable, and inclusive environment required.
- Computer skills required.
