Job Description

Our mission at Apicha Community Health Center is to improve the health of our community and to increase access to comprehensive primary care, preventive health services, mental health, and supportive services. We are currently seeking a Care Manager I to work at our 400 Broadway location.

Position Summary: The Care Manager will provide direct care management services to clients living with chronic diseases such as HIV/AIDS, substance use, mental illness, etc. This individual will be responsible for providing core services of Health Home care management for all clients that are assigned. They will perform an in-depth screening for functional scale eligibility, assessment, and reassessment of client needs that will include mental health, substance use, medical, financial, housing and other forms of support services.

Qualifications: Qualified applicant will have a bachelors degree in nursing, social science, psychology, social work, health or LPN with 1 year of relevant experience or AA/AS in health or human services field with 3 years of relevant work experience maybe considered equivalent of 1 year of bachelors degree. Bachelors degree in non-related field with 5 years of post-bachelors relevant work experience maybe considered. Experience in working with one of the following communities is required: chronic disease including HIV/AIDS; Substance user; Mentally ill; and Lesbian, Gay and Transgender (LGT); Fluency in French, Spanish or Asian language preferred; Excellent written and verbal communication skills in English; Ability to take public transportation is required; Ability to work flexible hours including some evenings and weekends.
