What youll do:
and nbsp;Conduct virtual and amp; inperson assessments to understand patients needs.
and nbsp;Cooperate with Priority Group Services to finalize clients.
and nbsp;Partner with patients families and amp; healthcare providers to create personalized care plans.
and nbsp;Educate and amp; empower patients on managing their health.
Youre a great fit if you have:
and nbsp;Active and unrestricted Registered Nurse (RN) license in the state of NJ.
and nbsp;2 year of clinical assessment experience (home care preferred)
and nbsp;Strong assessment and amp; communication skills
and nbsp;A passion for patient care and amp; teamwork
