Overview:

Key Responsibilities:

• Provide direct patient care in a home setting
• Administer medications and treatments
• Conduct health assessments and develop care plans
• Monitor and report changes in patient conditions
• Provide wound care and dressing changes
• Educate patients and families on treatment plans and health maintenance
• Coordinate care with other healthcare professionals
• Document patient care and progress
• Ensure compliance with healthcare regulations and standards
• Provide emotional support and reassurance to patients and families
• Collaborate with family members and caregivers
• Manage medical equipment
• Assist with activities of daily living
• Participate in multidisciplinary team meetings
• Stay updated on healthcare best practices

Required Qualifications:

• Registered Nurse (RN) license
• Bachelor of Science in Nursing (BSN) or equivalent
• Proven experience in a clinical setting
• Strong understanding of home care best practices
• Excellent communication and interpersonal skills
• Ability to work autonomously and in a team environment
• Proficiency in medication administration and wound management
• Compassionate and empathetic nature
• Knowledge of healthcare regulations and standards
• Ability to prioritize and organize patient care
• Critical thinking and problemsolving abilities
• Valid drivers license and reliable transportation
• Flexible schedule and availability for home visits
• CPR certification
• Ability to lift and move patients as needed
