Tips: Provide a summary of the role what success in the position looks like and how this role fits into the organization overall.
Responsibilities
Gathering information from outside sources (especially other care providers)
Assessing both behavioral and physical health conditions through a thorough history and physical examination
Capturing conditions not yet diagnosed including by ordering additional studies as indicated
Organizing the information in a manner that supports successful disability applications (if appropriate)
Conducting visits in the field or in an individual s home when needed
Making individualized recommendations and/or referrals for programs/care provided by firsthand or outside providers based on the assessed need
Lead the medical component of the teambased care provided including:
Training and advising firsthand Guides and Community Resources Guides on clinical issues and treatments Supporting Guide Team on care coordination
Leading care management identifying actionable care gaps
Performing medication reconciliation postdischarge and as needed
Qualifications
- Masters degree or higher
- 3 years of experience in ANY of the following:
- Primary Care
- Value Based Care
- Valid Nurse Practitioner (NP) in the state of Washington (WA)
- Home visits are a major component of this job. Are you okay visiting patients in their homes (yes)
- Do you have HCC Coding experience (yes)
coding experience,training and advising,home visits,conducting visits,leading care management,capturing conditions,organizing information,value based care,hcc coding,primary care,gathering information,assessing health conditions,performing medication reconciliation,supporting care coordination,nurse practitioner (np) certification,making recommendations
