Shift Notes: 5 days/week 40hr guarantee

Weekend Requirements: Every other weekend/ Holidays as needed

Traveler Policy: Will only consider Travelers with permanent address outside of the 50mile radius of the facility

The professional nurse providing care to medical/surgical patients utilizing all components of the nursing process (assesses develops plans implements and evaluates) in the plan of care.

Services Include:

• Skilled observation & assessment
• Wound care
• Injections & IV therapy
• Nursing Case Management
• Netsmart formally Allscripts is the EMR.

• Minimum of 2 years CURRENT Home Health experience in Case Management and Hospice Experience
• RN will be working in the Whitefield/Berlin NH area
• RNs will be providing Home Health & Hospice Case Management services to patients of all ages (infant to elderly)
• Netsmart/Allscripts EMR experience is strongly preferred
• Mileage paid at currently approved Federal IRS Rate (Mileage paid from the office each morning)

Profile Requirements:

• Work History
• Skills Checklist (pertinent to position) current to 1 year
• 1 Supervisory Signed Reference (from within the past two years of employment)
• License/Certifications Must be uploaded with profile

• BLS (AHA)

• New Hampshire

Compact Allowed

• NH or Compact RN license must be in hand at the time of upload. The facility will not pend license
