• Engage in initial and continuous outreach to high-risk patients, in-person to help educate them about the Our program and/or ensure they are accessing the program s benefits
  
• Communicate the benefits of the program to eligible members and overcome objections
  
• Drive extensively daily - most of the day is spent visiting community members, in their home and connecting them with care providers
  Provide warm hand-offs to the assigned clinicians and customer support when a member is agreeable to the program and ensure the initial enrollment appointment is scheduled and successfully completed
  

Requirements

• High School Diploma/GED
  
• Valid Regular Driver License (DL) license/certification
  
• 2+ years experience working in healthcare, sales, outreach, engagement, and/or enrollment campaigns
  
• Experience meeting or exceeding weekly quota goals
  
• Ability and willingness to be on the road most of the time
  
• Nursing home experience a plus
