Care Management Supervisor - MSW

Vivo HealthStaff is looking for an MSW to act as a Care Management Supervisor. In this role, the supervisor oversees care coordination and case management services, ensuring the integration of clinical services and adherence to program design. They also prioritize customer service, staff training, and proficiency in care coordination. The supervisor collaborates with the Program Manager, acts as a social work consultant, and provides education and advocacy to individuals and families. They handle caseloads, supervise and train staff, ensure compliance with legal and regulatory requirements, and prioritize safety and reporting. The supervisor upholds professional standards, practices according to HIPAA guidelines, and actively participates in process improvement and quality metrics. They adhere to policies, procedures, and safety guidelines.
