Job Description
General Summary
Interviews patients family and caregivers
Collaborates with outpatient facilities and pharmacies.
Works alongside the inpatient careteam in order to document the most accurate and uptodate list of medications a patient is taking at home.

Reporting Relationships
The Technician reports to the Clinical Pharmacy Coordinator and/or Pharmacist on duty. Job requires moderate level of supervision.
Qualifications :
Education: High School Diploma or equivalent required.
Completion of technician training program preferred.
Licensure: Registered Pharmacy Technician with State of Maryland Required. Certified Pharmacy Technician preferred.
Work Experience: One two years working experience as a pharmacy technician in a retail setting preferred or prior experience performing medication history/reconciliation in an institutional setting.
