Our client is seeking an Outreach Specialist to engage with community partners and drive referrals for the PACE program in Los Angeles. The ideal candidate will have experience in healthcare sales geriatric experience and preferably bilingual. This individual educates the community partners prospective participants families and or caregivers about the PACE program. The Outreach Specialist helps keep the frail elderly to live independently at home and continue to enjoy the quality of life.
- Full Time Permanent Position for An Outreach Specialist
- Hybrid Infield work 23 days/week Onsite at Clinic Center 23 days/week
- Monday Friday 8AM5PM

Responsibilities
- Develop and maintain relationships with healthcare professionals and community partners
- Collaborate with Enrollment Specialist to ensure successful enrollment
- Organize and conduct events to promote the PACE program
- Maintain CRM software records
- Adhere to DHCS and CM regulations
- Develop marketing plans for enrollment
- Collaborate with management on retention strategies
- Attend community and healthcare events to increase census
- Identify opportunities to enhance referral process
Requirements
- Bachelor s Degree or Higher
- 2 years experience in Community Relationships and Geriatrics
- Healthcare Sales experience
- Must have reliable transportation good driving record and auto insurance
- Bilingual in English and any of the following languages: Spanish Korean Cantonese Mandarin or Vietnamese.
