Title Care Manager I (RN)

Duration 6 months

Shift Mon to Fri 8 am to 5 pm

Location Remote KS

Walk me through the daytoday responsibilities of this the role and a description of the project :

• Check email and voice mail for important communication and member needs.
• Communicate with providers as needed.
• Address member needs as they arise.
• Plan/schedule inperson visits with members.
• Complete required monthly telephonic outreach to members
• Complete all necessary documentation in EMR.
• Travel to member homes biannually and perform assessments required documentation.
• Attend required meetings.
• Work across teams to solve member issues.

Describe the performance expectations/metrics for this individual and their team:

• Inperson visits completed within 180 days
• All required documentation completed within 10 days of visit/phone call.
• Signatures from parents obtained within 30 days of the meeting date.
• Compliance with all other state requirements per manager direction
• Punctuality

Required Skills/Experience:

• RN Experience at least 3 years
• Pediatric RN experience
• Experience with Electronic Medical Record computer skills.
• Meticulous.
